Dental Volunteer Forms

 

Please find three forms below. The data from these forms will be added to the ADA Volunteer database. We would ask that if any details change, please let us know.

Volunteers

To have your name added to the ADA Inc. volunteer database, please fill out this form and 1) submit it online, or 2) mail it or 3) fax it to the address listed below. When a project coordinator makes inquiries about potential volunteers that correspond with your information, you will be sent a brief e-mail about that project and the e-mail address of the project coordinator. You will be able examine the details of projects on the Volunteer Project page. Please note there is no expectation that you have to join any project. You will be asked to update this information annually.

Projects

If you are a project coordinator of an Australian dental volunteer project, please fill out this form and 1) submit it online, or 2) mail it or 3) fax it to the address listed below. The form is self-explanatory and will be able to be viewed by potential volunteers on the Volunteer Projects page. Space is available to place a hyperlink for your project web site. You'll be asked update this information each six months.

Donations

If you have material, equipment, books or anything else that you wish to donate, please read the following information about donations. Fill out the form with as much information as possible and 1) submit it online, or 2) mail it or 3) fax it to the address listed below. The information submitted will be sent to all Project Coordinators registered with the ADA. They will contact you for additional details if they are interested what you have to donate.

Instructions:

The PDF forms can be opened with Acrobat Reader. If you use Acrobat Reader 7 or later, the forms may be filled out online, printed out (for your records) and submitted online. We strongly advise updating to the latest version of Acrobat Reader by clicking on the icon below. Alternately, you must print out the form(s), fill them in and then mail or fax a copy to:

The interactive forms are Mac and PC compatible. When filling out the forms online:

 * You must type only in the spaces provided. 
 * You can cut and paste from another source document e.g. MS Word. 
 * On completion, the form can printed out for your records.
 * To submit automatically, click on the 'Submit', button. This will:
        - open up a new message in your default email client 
        - create a data file of your responses
        - attach the data file to the new email message addressed to the
          ADA Inc.
 * Click 'Send' message

Again, you must use Acrobat Reader 7 or later in order to fill the forms out and submit online. Dental Health Week is a community awareness program coordinated and funded by the ADA. In 2009 the ADA is leading a campaign to target 55 year olds and over and has developed a range of Fact Sheets on oral health care. August 3rd - 9th is Dental Health Week.
